ELIGIBILITY

You are eligible for insurance 1st of the month following 30 days of full time employment. 

You may enroll in the benefits when you are first eligible.  You are only eligible to make additions, terminations and changes to your enrollment during the year if you have a change in family status or at annual open enrollment.

Changes in Family Status:
  • Marriage
  • Divorce
  • Death
  • Birth
SECTION 125 CAFETERIA PLAN

  • Hardeman County offers a Cafeteria Plan to employees following 30 days of full time service.  
  • Most premiums paid by employees will be deducted on a pretax basis.
  • You have the option of electing to have a portion of your salary redirected into the Unreimbursed Medical Account to be used whenever you or your dependents incur medical expenses that are not covered by medical insurance such as over the counter medication, co-pays, deductibles, and coinsurance.
  • You can also have pre-taxed money redirected to a Dependent Care Account.
  • Be conservative when allocating money into the plan because if you do not use the money by the end of the plan year, you will lose it.  You will have 90 days following the end of the year to submit your expenses.

STANDARD LONG TERM DISABILITY
All full time employees can enroll in the Long Term Disability Plan.  If an employee becomes disabled, the plan will pay a benefit of 60% to a monthly maximum of $6,000 once the elimination period has been satisfied.  The elimination period is 180 days.  This is an employer paid benefit.
STANDARD LIFE INSURANCE
Group Life - Employer Paid
All full time employees can enroll in the Group Life and AD&D insurance.  The benefit for each employee is 1 times Annual Earning up to $50,000.  This is an employer paid benefit.

Voluntary Life Insurance
You may elect Additional Life coverage in units of $10,000.  New hires have a one time opportunity to purchase up to $100,000 without medical questions.  You may also elect coverage for your spouse & kids.
MEDICAL INSURANCE
Hardeman County Memorial Hospital currently contracts with AETNA Insurance Company for employee health insurance coverage.  Employees have two different options for health insurance plans. 

The first option is the Base Plan #E256:
  • Office Visit Co-pays;  ($40 PCP) and ($50 Specialists)
  • Deductibles;  Network ($2500 Individual) and ($5000 Family), Out of Network ($5000 Individual) and ($10,000 Family)
  • Coinsurance;  Network (70%) and Out of Network (50%)
  • Out of Pocket Maximums including Deductibles;  Network ($7500 Individual) and ($15,000 Family), Out of Network ($15,000 Individual) and ($30,000 Family)
  • Prescription Drugs;  Generics ($15 copay), Brand Formulary ($35 copay), Brand Off Formulary ($50 copay)
The second option is the Buy-Up Plan #E106:
  • Office Visit Co-pays;  ($25 PCP) and ($35 Specialists)
  • Deductibles;  Network ($1000 Individual) and ($2000 Family), Out of Network ($2000 Individual) and ($4,000 Family)
  • Coinsurance;  Network (80%) and Out of Network (20%)
  • Out of Pocket Maximums including Deductibles;  Network ($4000 Individual) and ($8,000 Family), Out of Network ($8,000 Individual) and ($16,000 Family)
  • Prescription Drugs;  Generics ($15 copay), Brand Formulary ($35 copay), Brand Off Formulary ($50 copay)
