Hardeman County Memorial Hospital currently contracts with Blue Cross Blue Shield Insurance Company for employee health insurance coverage. Employees have two different options for health insurance plans.

The first option is the BCBS 1000:
Office Visit Co-pays:($25 PCP), ($55 Specialists/Urgent Care), ($100 ER)
Deductibles: Network & Out of Network ($1000 Individual) and ($3000 Family)
Coinsurance: Network (75%) Out of Network (55%)
Out of Pocket Maximums: Individual in Network ($5,000), Individual Out of Network ($9,000)
Family in Network ($15,000), Family Out of Network ($27,000)
Prescription Drugs: Generic ($20 copay), Brand Name ($40 copay), Non-Preferred Brand Name ($60 copay)



The second option is the BCBS 3000:

Deductibles: Network & Out of Network ($3000 Individual) and ($9000 Family)
Coinsurance: Network (80%) Out of Network (60%)

Hardeman County offers a Cafeteria Plan to employees following 30 days of full time service.
With this plan you can have a portion of your salary redirected into the Unreimbursed Medical Account to be used for you or your dependents for expenses not covered by the medical plan such medications, copays, deductibles and coinsurance.
You can also have pre-taxed money redirected to a Dependent Care Account.

Group Life and AD&D insurance.The benefit for each employee is 1 times Annual Earning up to $50,000.This is an employer paid benefit.

You may elect Additional Life coverage in units of $10,000. New hires have a one time opportunity to purchase up to $100,000 without medical questions.You may also elect coverage for your spouse & kids.

All full-time employees will be enrolled in both the Long Term Disability (LTD) & Short Term Disability (STD) plans. This is paid by the Employer.

LTD - If an employee becomes disabled, the plan will pay a benefit of 60% to a monthly maximum of $6,000 once the elimination period has been staisfied. The elimination period is 180 days.

STD - If an employee becomes sick or disabled for 14 consecutive days, the plan will pay a benefit of 60% of salary (not to exceed $1,500 per week). The benefit duration for Short Term Disability is 180 days.
Hardeman County offers a 401K plan to employees 21 and older after they have completed 3 months of full time employment. Voluntary products are also offered through Colonial Insurance.

In Network:
Eye Exam paid in full every 12 Months. All lenses paid in full every 12 Months. Frames paid in full every 24 Months.

Out of Network:
Up to a $45 reimbursment.
